Before invading the uterine wall (implantation), a five-day embryo (at the blastocyst stage) must independently be released from the surrounding transparent membrane. This process is called hatching. Sometimes the embryo cannot be released from its own membrane and implantation does not occur, in these cases, auxiliary hatching (dissection of the transparent membrane) can increase the likelihood of pregnancy. In these cases, the biological parents of the child are considered to be the couple who gave the egg and sperm for the formation of the embryo, and the woman who carried their child is called a surrogate mother.

ICSI (IntraСytoplasmic Sperm Injection) - “the introduction of a sperm into the cytoplasm of the egg”, is one of the auxiliary stages of IVF, which is recommended in the presence of male factor infertility. The essence of the method is that the embryologist selects the most promising spermatozoa for fertilization and with the help of a thin needle introduces them into the eggs.

IMSI (Intracytoplasmic Morphologically Selected Sperm Injection) and PICSI (Physiologic Intra Cytoplasmic Sperm Injection) - modifications of the ICSI method, more advanced methods of sperm selection for injection into the egg cell.

Artificial insemination is one of the methods of achieving pregnancy, successfully used in reproductology for many years. The essence of the method is that sperm (spouse or donor) is introduced through the cervical canal into the uterus of a woman. The further process of fertilization of the egg by the sperm occurs naturally. Artificial insemination can be carried out in the natural menstrual cycle, or after stimulation of ovulation. The pregnancy rate after intrauterine insemination is up to 17% and depends on a number of concomitant factors.

Cryopreservation is a method of freezing and storing sperm, eggs and embryos with the possibility of complete restoration of their biological functions after defrosting. Preimplantation genetic diagnosis of embryos (PGD) is a method for diagnosing the chromosomal pathology of embryos obtained as a result of IVF, which can significantly increase the frequency of pregnancy and also prevent the birth of a child with a hereditary disease. Indications for PGD are: carriage of chromosomal rearrangements, a woman over 35 years old, a history of more than three spontaneous abortions in the early stages, or more than two or three failed IVF attempts.

Today, there are several methods of preimplantation diagnostics. The first of them, genetic screening by the FISH method, allows you to analyze 3-12 pairs of chromosomes, determine gender and exclude the most common genetic diseases: Down syndrome, Edwards syndrome, Shereshevsky-Turner syndrome, etc. The second is a method of comparative genomic hybridization on microarrays ( aCGH), is becoming increasingly common in modern clinical practice and makes it possible to check all 23 pairs of chromosomes in the embryo. Great prospects are associated with the method of genomic sequencing (NGS). The essence of preimplantation genetic diagnosis comes down to the ability to choose an embryo without chromosomal pathology. The study of embryonic cells is carried out on 3-5 days of its development, depending on the diagnostic method. According to the results of the study, geneticists can confirm the presence or absence of chromosomal abnormalities in embryos. To determine the need for PGD, you must make an appointment with a geneticist and undergo medical and genetic counseling.

Modern technologies of freezing (vitrification) of cells allow you to save the genetic material of patients planning a future pregnancy, as well as for those who are faced with diseases whose treatment can negatively affect the ability to have children. The preservation of oocytes and sperm makes it possible to postpone the decision on the issue of planning pregnancy and offspring (even in the youngest patients) until a more appropriate moment. Lack of ovulation (anovulation) is one of the main causes of infertility. This is a condition in which a certain examination and treatment is required aimed at eliminating the identified cause of anovulation. Why is this program also called Risk Sharing?

Obviously, if you become pregnant already in the first cycle of IVF with a guarantee, then you will pay for this pregnancy more than for one regular IVF cycle.

If you become pregnant after several IVF cycles and several transfers of thawed embryos, your costs will be significantly lower than the costs of conducting standard IVF cycles, since you will not pay for cryopreservation of embryos, transfers of thawed embryos, ICSI and other additional services. If all treatment cycles are unsuccessful, then the clinic conducting it will have to reimburse the cost of all failed IVF attempts. This is the financial risk that you share with the clinic.

What are the criteria for selecting patients to participate in the IVF with Guarantee program?

Typically, 75% of patients under the age of 35 have a high chance of becoming pregnant after IVF and meet all the necessary requirements for participating in this program. Therefore, criteria have been developed by which you can enroll in the program in Russia or abroad. Each clinic has the right to require that a preliminary examination and analysis be carried out in its laboratory.

General requirements:

  • Age under 35 at the start of the program.
  • Less than 3 failed IVF attempts in history.
  • The presence of an extract on previous cycles of in vitro fertilization in the presence of a history of previous IVF attempts.
  • The patient must be non-smokers or quit smoking at least three months before the start of the IVF cycle, which will be confirmed by analysis.
  • Lack of obesity. Body mass index should not be higher than 29 (body mass index is calculated as weight in kilograms divided by height in meters squared).
  • Consent to conduct ICSI (if indicated), transfer 2 or 3 embryos into the uterine cavity and cryopreserve (freeze) the remaining viable embryos (cryopreserved embryos should be transferred to the uterine cavity before the start of the next IVF cycle).


Results of the analysis of ejaculate (sperm should indicate normozoospermia according to WHO criteria (at least 5,000,000 motile sperm in the ejaculate (of which active motility is at least 25%)).

Ovarian Function:

  • The level of FSH in the blood on the 2-3 day of the cycle should not be higher than 9.0 IU / l, and the level of estradiol should not be higher than 80 pg / ml.
  • The absence of cases of increased levels of FSH in the blood above the norm in the anamnesis (even a single one).
  • The presence of two ovaries available for transvaginal puncture, and at least 10 antral follicles in both ovaries.

The condition of the uterus and fallopian tubes (according to expert ultrasound):

  • The thickness of the endometrium on the 13-14th day of the menstrual cycle is at least 8 mm.
  • Lack of hydrosalpinxes.
  • The absence of myomatous nodes of any localization larger than 2.5 cm in diameter.
  • The absence of submucous myomas, polyps and adhesions in the uterine cavity.
  • The absence of intramural fibroids that deform the uterine cavity.
  • No barriers to transcervical embryo transfer.
  • The absence of congenital malformations or acquired deformation of the uterine cavity, in which implantation of the embryos or gestation of the pregnancy is impossible.
  • If there are pathologies of the uterus or hydrosalpinges, the patient can be included in the IVF with a guarantee program after surgical correction of these pathologies.
